A Geriatric Mental Health Nurse is a licensed practical nurse with extensive experience and expertise in dealing with mental health issues. Our nurse provides a link for seriously and persistently mentally ill adults with the appropriate medical and social services and resources within the community.
Our Program serves individuals who are 50 years of age and older who are homebound and have a history of serious and chronic mental illness. Participation in the program is through referral only, and the person must be willing to engage in the program. There is no fee for this program.
The goal of the Geriatric Community Mental Health Nurse Program is to provide people with the appropriate services and treatment that will ultimately delay or prevent hospitalization. This is accomplished through assessment and evaluation of the person's needs. The nurse also works closely with the person's family, and other professionals, and agencies to design an appropriate plan.
Referrals are accepted on a first-come, first-served basis from families, area hospitals, physicians, service providers, and community agencies. People usually remain in the program for six to ten weeks.
